Why remove them

When a wisdom tooth should come out

Healthy wisdom teeth that have enough room and aren’t causing problems can stay. We recommend removing them when they are, or are likely to.

Protecting the tooth in front

A wisdom tooth growing at an angle can dig into the back of the tooth in front of it, trapping food and causing decay that’s hard to clean or repair. Left too long, it can mean losing that tooth as well.

Repeated infections

A partly erupted wisdom tooth can trap food and bacteria under the gum flap, causing painful, swollen infections (pericoronitis) that keep coming back.

Decay

Wisdom teeth are hard to reach with a toothbrush, so they often decay, and are often too far back to repair well.

Pain and pressure

Ongoing pain or pressure at the back of your mouth, or discomfort when you open or chew.

Gum disease

Food and plaque trapped around a wisdom tooth can cause gum disease that affects the teeth next to it.

Cysts

Rarely, a fluid-filled cyst can form around a buried wisdom tooth and damage the bone.

Wisdom teeth removed at the practice

Almost all wisdom teeth are removed here at the practice. Straightforward ones that have come through, often lower wisdom teeth, can be removed by your own dentist, and Dr Rohan carries out surgical removals, including many impacted teeth (stuck under the gum or against the tooth in front), so you don’t have to wait for a hospital appointment.

Before any removal, he checks the position of the tooth and its roots with X-rays and, where needed, a 3D scan at a specialist imaging centre, at a preferred rate for our patients. If a tooth ever needs hospital care, he’ll tell you honestly and arrange the referral.

Dr Rohan Shaffery

Dr Rohan has advanced training and a special interest in oral surgery, implantology and periodontology. After graduating from King’s College London, he completed a Master’s in Clinical Implantology and became a Member of the Faculty of Dental Surgery at the Royal College of Surgeons of Edinburgh.

He carries out our surgical wisdom tooth removals here at the practice, including many impacted teeth, and will always tell you honestly whether a tooth needs to come out.

Coronectomy

When a tooth is very close to the nerve

What it is

Instead of removing the whole tooth, Dr Rohan removes the crown and leaves the roots in place, where they usually heal over with bone.

Why it helps

For a lower wisdom tooth whose roots sit very close to the nerve, it greatly reduces the risk of nerve damage, while still removing the part causing problems.

What to expect

  1. Assessment

    We examine your wisdom teeth and take X-rays, arranging a 3D scan if needed, and explain whether removal is recommended, how, and who will do it.

  2. Comfort

    The area is thoroughly numbed, or sedation with a visiting anaesthetist can be arranged if you’re nervous.

  3. Removal

    The tooth is removed gently, sometimes in sections through a small opening in the gum, which is then closed with a few stitches.

  4. Recovery

    Most people feel sore and a little swollen for a few days, and back to normal within about a week.

  5. Review

    We see you for a review to check you’re healing well. Your review appointments are included in the price.

Recovery

After your wisdom tooth removal

Swelling and soreness

Swelling usually peaks after two or three days. Cold packs on your cheek and everyday painkillers, as advised, help.

Protect the clot

For the first 24 hours, avoid rinsing, hot drinks, straws and smoking, so the clot stays in place.

Soft foods

Eat soft foods for a few days, and from the next day rinse gently with warm salt water after meals.

Call us if

Pain gets worse after a few days, you have a bad taste, a fever or swelling that keeps growing. It could be a dry socket or infection, which we can treat.

Do I need my wisdom teeth out?

Only if they’re causing problems, or are likely to. Healthy wisdom teeth with enough room can stay. Dr Rohan will check yours and explain what he recommends.

What is a coronectomy?

Removing the crown of a wisdom tooth and leaving its roots in place. It’s used when the roots are very close to the nerve, to reduce the risk of nerve damage.

Will it hurt?

The area is numbed first, so you’ll feel pressure but not pain. Sedation with a visiting anaesthetist is available if you’re nervous. Soreness and swelling afterwards usually ease within a few days.

How long is the recovery?

Most people are back to normal within about a week. Swelling usually peaks after two or three days.

Can I be sedated?

Yes. Sedation with a visiting anaesthetist can be arranged on request.

Should I have all four removed at once?

Not necessarily. We only remove the wisdom teeth that need it, sometimes at the same appointment, and Dr Rohan will talk you through the options.

What if my wisdom tooth is infected now?

Call us. We’ll see you the same or next working day to treat the infection, and plan removal once it has settled if needed.
